Service Description: Onsite Transitional Housing: eleven fully furnished efficiency units for homeless men and women. Scattered Site Transitional Housing: 15 units located within the Fox Cities for homeless families and individuals. Rapid Re-Housing: Scattered Site subsidized housing for 16 families and individuals.Eligibility: Homeless single adults and families. Intake procedure:Referral from other agency or call main number. Hours: Monday-Friday 8:00 AM - 4:00 PM Languages: English Area Served: Outagamie County and designated areas in Winnebago/Calumet counties.
