Unity Care offers housing solutions that provide youth and young adults a safe and secure place to live as they focus on their emotional, educational, and employment goals. They integrate resources and support services to ensure the well-being of those they serve, and create a home-like environment where youth and families feel protected. THPP provides current foster youth (ages 16-18) the necessary life skills to live independently upon exiting the child welfare system. Youth receive round-the-clock on-site support and supervision from the THPP staff and county social workers. They also connect them to Independent Living Programs in Santa Clara County.Their mission is to provide quality youth and family programs for the purpose of creating healthier communities through lifelong partnerships Hours: Monday - Friday 9 am - 5 pm

Mission Statement: Unity Care’s mission is to provide quality youth and family programs for the purpose of creating healthier communities through lifelong partnerships. Our goals are to provide safe, secure, and positive living environments; educate and support underserved youth; and prepare young people to pursue professions in which minorities are chronically underrepresented.
