The Transition House provides transitional housing combined with supportive services to ten disabled homeless person in the City of Fall River. Case Management services provided at the Transition House connect clients to housing, employment, entitlements, behavioral health, and and array of other services. Clients become engaged in all supportive services needed and as a result, successfully transition into the community and experience housing stability and improved health and quality of life.
