Lakes and Prairies provides supportive services to homeless families/individuals who meet the eligibility criteria, have a strong desire to break the cycle of homelessness and are willing to work on specific goals.
These programs focus on families and individuals who typically have a difficult time reaching self-sufficiency due to one or more of the following issues:
mental illness, domestic violence, chemical dependency, unlimited independent living skills, and/or lack of marketable skills/education.
Clients are provided with the support and guidance necessary to maintain stable housing.
Services are based on the family or individual specific needs.
They include but are not limited to:
* Case Management.
* Referrals (Employment, Health, Food, Clothing, Social Services...).
* Education.
* Advocacy.
* Financial assistance.
* Transportation
